To fill the application form, use the following steps:

Click on the View Application Form at the top right of this page or under students section, click View Application Form
On the new page, click the type of application you want to fill.
Enter your access code/PIN and Entry Registration Number to make payment and Fill your form.
Click Submit and Print your acknowledgement slip.
